Applications
Individual Membership (B-1) - This application is for those who wish to have access
to the information that is communicated within the PIE Cooperative Network
and/or wish to become a PIE Course Evaluator. (updated 12/09)
Program Organization - This grade of membership is no longer being offered. (updated
11/07/05)
Sponsor Organization (B-12)- This is a preliminary application for organizations that have the
procedures and capability to prepare, promote, and evaluate PDH
hours of training through a variety of CPC activities in accordance with PIE and NYSED requirements.

use in conjunction with an approved course to record attendance for proper
